A long time ago our gadgets learned to monitor our health and give a signal about a problem if there is any. An owner of a private clinic aiming to fuze the bases of two healthcare information systems HIS into one contacted us. Two institutions using different systems for patient accounting and data processing were to be merged. Each clinic had developed its system on its own account over 7 years ago. The leadership of the new fuzed medical center was content with both systems, and it was up to us to decide which one to eliminate. Our main goal was to save the most valuable data. The two programs were very similar in terms of functionality, but under the hood, in the bases, there was nothing in common. We decided not to reinvent the wheel on the basis of two old ones but to put everything into a new base to which we adapted one of the two reception desks. In favor of it, we can count the fact that Apple Inc. HL7 is a standard for the exchange, management, and integration of electronic medical information.

The project is described in detail in this thesis in Slovenian language. There is also no change in the database itself - all data is saved in the same way and no change in the queries is needed, even though some data in the DB is encrypted. So, even if an attacker gets a full access to the DB, the data is useless, because the resources are impossible to be interpreted due to encrypted Patient reference. The microservice also enables to change the encryption key for the patient see Encryption Key Change. The main goal of this solution is, that there are no changes needed in the eHealth app except for changing the access point to Crypto Microservice. There aren't any significant changes in the FHIR server. The only difference is, that in this setup, it communicates with Crypto Microservice AP instead of with the eHealth app directly. Patient reference is encrypted before sending the Observation to the server.
Interoperability is a big discussion in health care, with new regulations requiring interoperability for patient data. Yet Health Level Seven HL7 , with its new Fast Healthcare Interoperability Resources FHIR standard for the electronic transfer of health data, is leading to a rash of implementations that, to date, are not solving core interoperability issues. The way out is not to add more middleware, but to upgrade the basic tools of interoperability in a way that finally brings healthcare technology into the 21st century.
